The NHS Discharge Medicines Service is an essential service for community Pharmacy’s in the UK.

This service has been established to ensure better communication between primary and secondary healthcare settings.

This means that if a patient has been in hospital and there have been changes made to their medication, this information  is passed onto their community pharmacy on discharge to prevent harm and readmissions. 

Here at the pharmacy we then dispense the correct medication and are able to have a consultation with our patients to ensure they understand the changes made and to ensure they return their unwanted medication that may have been stopped, to avoid any confusion.
